A 66-year-old male with a history of long-standing ulcerative colitis, managed with azathioprine and sulfasalazine, underwent a flexible sigmoidoscopy for a flare up of his symptoms. Endoscopy showed essentially normal mucosa (Mayo Score 0) but a fibro-epithelial polyp was observed at the ano-rectal junction and biopsied. No synchronous lesions, nor further polyps were detected on completion of the pancolonscopy. Histology from the biopsy showed high-grade dysplasia (HGD) with some features which raised the possibility of deeper invasion.
