A high grade intraepithelial neoplasia in which there is marked architectural distortion and crowding of the glands, associated with severe cellular atypia. The neoplastic cells display pleomorphic and hyperchromatic nuclei.

This lesion shows moderate or marked architectural distortion with glandular crowding and prominent cellular atypia. It includes moderate dysplasia and severe dysplasia. (WHO, 2000)
