A standardized 36-item questionnaire, administered by a friend, relative, or caretaker, that utilizes a 5-point rating scale to assess an individual\'s ability to perform activities in six domains of functioning over the 30 days prior to evaluation in order to calculate a global disability score. The questionnaire was developed in 2010 by the World Health Organization (WHO) Classification, Terminology and Standards team, within the framework of the WHO/National Institutes of Health (NIH) Joint Project on Assessment and Classification of Disability.
