The surgical team implements a standardized briefing process before the surgical procedure to include the introduction of team members, identification of the correct patient, procedure, site, and side as well as patient risks and procedure concerns are addressed. The site is marked by a licensed independent practitioner who is accountable and present when the procedure is performed. A time out is performed before the procedure begins and activities are suspended to the extent possible so that team members can focus on active confirmation of the correct patient, site and procedure to be performed. A debriefing is performed to discuss counts, procedure performed, labeling and disposition of specimens, equipment and instrument issues, missing supplies, VTE prophylaxis, would classification and other safety concerns.
